Aurora Care Management
Care Management is a focused, coordinated approach to delivering care across a broad range of service locations, by many different providers of care, at many different points in time. In 1995, Aurora Health Care confirmed its commitment to clinical excellence through the implementation of an initiative designed to provider coordinated, high quality service to people with asthma. This was the beginning of the care management philosophy that has become the core of Aurora's mission, vision, and values.
Over the years we have continued to find better ways to address numerous health care issues through care management/quality initiatives. This site is a quick snapshot of Aurora's major system-wide initiatives focused on prevention and wellness, acute illness, and chronic disease. These initiatives allow us to be more proactive; to help prevent health problems, as well as to treat them; and to use proven "best practices" consistently, wherever care is provided.

We will be satisfied only when Aurora gives people better access, better service, and better results than they can get anywhere else. We hold ourselves accountable to the communities we serve and strive to exceed national standards. We are committed to being transparent with our care management/quality efforts and will share our results with the Centers for Medicare and Medicaid Services, the American Hospital Quality Initiative, the Joint Commission on Accreditation of Healthcare Organizations, the Leapfrog Group, and other national organizations and initiatives.
