Paige Black, PharmD

Clinical Pharmacist Paige received her PharmD, along with a minor in Religion and concentration in Diabetes Management, from Drake University in Des Moines, Iowa. She completed her PGY1 Pharmacy Practice Residency with North Shore Long Island Jewish in Manhasset, New York. She then went on to complete the ASHP Pharmacy Leadership Academy. She is currently a clinical pharmacist with the Central Order Verification (COV) Pharmacy and co-preceptor for the PGY1 Resident COV rotation.

Kristin Cannon, RPh

Sr. Director Pharmacy Operations for Aurora Health Care - North Kristin received her B.S. in Pharmacy from the University of Wisconsin. Kristin is the Sr. Director of Pharmacy Operations for Aurora Health Care-North, which includes the 6 hospitals north of the Milwaukee area and the Aurora Central Order Verification (COV) Pharmacy. Kristin co-precepts the administration rotation at Aurora BayCare Medical Center and is a preceptor in the PGY2 Health System Pharmacy Administration program

Jennifer A Esch, PharmD, BCPS

Clinical Pharmacist Jennifer completed her undergraduate work at Michigan State University with dual Bachelor's degrees in Business Management and Education. She obtained her Doctor of Pharmacy degree from the University of Arizona. She currently precepts the Medical/Surgical Clinical and Hospitalist required rotations at Aurora BayCare Medical Center in Green Bay.

Kimberly Johnson, PharmD, BCPS

Specialty Pharmacy Coordinator - Centralized Order Verification Pharmacy Kim received her B.S. and PharmD from The University of Toledo in Toledo, Ohio. She completed a PGY1 Pharmacy Practice Residency at Aurora Health Care. She is currently the Clinical Coordinator for the Centralized Order Verification Pharmacy and serves as the 2017 chair of the Aurora Pharmacist Practice Council. Kim precepts the elective APPE Remote Medication Management student rotation and co-precepts the PGY1 Resident COV rotation as well as the elective APPE Neonatal ICU rotation.

Julie L. Kindsfater, PharmD, BCPS

Medication Safety Program Manager Julie received her PharmD from the University of Minnesota - Twin Cities. She completed a PGY1 Pharmacy Practice Residency at Froedtert Memorial Lutheran Hospital in Milwaukee. Julie co-precepts the drug policy rotation and assists residents with medication safety projects. She chairs the Aurora Medication Safety Committee and recently completed coursework in Patient Safety Leadership through the College of Medicine at the University of Illinois - Chicago.

Holly Kaminski, RPh

Clinical Pharmacist Holly received her B.S. in Pharmacy from the University of Wisconsin-Madison. She joined Aurora Health Care Metro Region in 2000 and moved North to Aurora BayCare Medical Center in 2002. Holly's primary focus is central pharmacy distribution and order verification for BayCare Medical Center. She precepts the Central Order Verification (COV) rotation in collaboration with Aurora Health Care's Metro Verification team

Jessie Lawton, PharmD, BCOP

Clinical Pharmacist Jessie received her BS from Marquette University and her PharmD from the University of Minnesota. She completed a hematology/oncology specialty residency at NorthShore University Health System in Evanston, IL. Jessie practices at Aurora BayCare Medical Center and precepts for the PGY1 oncology rotation and PGY2 oncology residency.

Veronica Maher, PharmD

Clinical Pharmacist Veronica received her PharmD from UW Madison and completed her PGY1 pharmacy residency at Marshfield Clinic in Marshfield, WI

Tracy Mingori, PharmD, BCCCP

Clinical Pharmacist Tracy received her B.S. in Pharmacy from Ferris State University and her PharmD from the University of Florida. She completed a PGY1 Pharmacy Practice Residency and PGY2 Critical Care Residency at Spectrum Health in Grand Rapids, Michigan. Tracy currently co-precepts the Adult Medicine and Critical Care rotations at Aurora BayCare Medical Center.

David Reeb, PharmD, BCPS

Clinical Pharmacist David received his PharmD from Drake University in Des Moines, IA. He completed a PGY1 residency at Gundersen Health System in La Crosse, WI. He now precepts residents on their Medical/Surgical and Hospitalist rotations at Aurora BayCare Medical Center.

Sara Reeb, PharmD, MBA, BCPS

Clinical Pharmacist Sara received her PharmD and MBA from Drake University. She completed a PGY1 Pharmacy Residency at Aurora St. Luke's Medical Center. She precepts the PGY1 Medical Surgical Rotation and Hospitalist Rotation.

Lee received his PharmD from the University of Wisconsin-Madison. He completed a PGY1 Pharmacy Practice Residency and a PGY2 Critical Care residency at Barnes-Jewish Hospital in St. Louis, MO. Lee co-precepts the critical care rotation and infectious diseases elective.

Karrie Stanke, PharmD, BCPS

Clinical Pharmacist Karrie received a B.S. in Biology and PharmD from UW - Madison. She completed a PGY1 Pharmacy Practice Residency at Aurora St. Luke's and has been practicing at Aurora BayCare since 2016.

Lisa Streu, RPh

Clinical Pharmacist Lisa received her B.S. in Pharmacy from the University of Wisconsin-Madison. She has been practicing at Aurora Bay Care Medical Center since 2005 and co-precepts the oncology rotation.

Jeff Waise, PharmD, MBA

Director Inpatient Pharmacy Jeff Waise has been the Pharmacy Director at ABMC since 2008. He is a PharmD graduate of University of Michigan with almost 20 years of management experience. He recently obtained his MBA in 2016. Under Jeff's leadership, ABMC has gone through 3 successful Joint Commission surveys, converted to an All Meds Model, implemented automation, and converted to Epic. Additionally, he has lead several successful clinical interventions to include parenteral nutrition, warfarin dosing and counseling, and transitions of care. Jeff is passionate about medication safety and continued process improvements.

Paul Windisch, PharmD

Specialty Pharmacy Coordinator, Drug Policy Paul received his B.S. in Economics & Statistics from University of Minnesota and his PharmD from the University of Nebraska Medical Center. He completed a PGY1 Pharmacy Practice Residency at University of Wisconsin Hospital & Clinics, and completed a two-year fellowship in pharmacoeconomics, technology assessment & outcomes research at University Health System Consortium (UHC). Paul co-precepts the drug policy rotation.
