Erika Aldag, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Abdominal Transplant Erika received her Pharm.D. from Drake University. She completed her PGY1 Pharmacy Practice residency at the University of Nebraska Medical Center and herPGY2 Solid Organ Transplant residency at The Ohio State University Medical Center. She precepts both the PGY1 and PGY2 Critical Care Abdominal Transplant rotations.

Audra Andersen, Pharm.D.

Clinical Pharmacist Audra received her Pharm.D. from the University of Wisconsin-Madison. She completed her PGY-1 residency at Aurora St. Luke's Medical Center. She currently practices at the Aurora Cancer Care outpatient oncology clinics.

Todd Armbruster, Pharm.D.

Clinical Pharmacist Todd graduated from Drake University. Todd practices and precepts in the intensive care unit at Aurora Sinai Medical Center.

Dennis Brierton, Pharm.D., BCPS, FASHP

System Director, Clinical Services Dennis received his B.S. from the University of Wisconsin and his Pharm.D. from the University of Illinois-Chicago. He completed a specialty residency in adult internal medicine at Purdue University.

Emily Bryant, PharmD

Clinical Pharmacist Emily completed pre-pharmacy coursework at the University of Wisconsin prior to receiving her Pharm.D. from Concordia University Wisconsin. She completed her PGY-1 Pharmacy Residency at Aurora St. Luke's Medical Center. Emily currently practices at St. Luke's Medical Center where she helps precept PGY-1 residents on medical and surgical rotations.

Jane Bubik, Pharm.D.

Investigational Drugs Coordinator Jane received both her B.S. and Pharm.D. from Purdue University. She completed a PGY-1 Pharmacy Practice Residency at Rush-Presbyterian St. Luke's Medical Center in Chicago. Jane offers an elective investigational drugs rotation.

Angela Colella, PharmD

Drug Formulary Specialist Angela received her B.S. in English Literature from the University of Wisconsin Madison and her Pharm.D. from Concordia University Wisconsin. She completed a PGY-1 Pharmacy Practice Residency at Aurora Health Care, Metro, Inc. A co-preceptor of the drug policy rotation, Angela also staffs at Aurora St. Luke's Medical Center.

Margaret Cook, Pharm.D., BCPS

Specialty Pharmacy Coordinator Margaret received her B.S. from the University of Wisconsin and her Pharm.D. from the University of North Carolina. She completed a PGY-1 Pharmacy Practice Residency at Hartford Hospital, Harford, Connecticut. Margaret practices at Aurora St. Luke's Medical Center. She co-precepts rotations in infectious diseases for PGY1 and PGY2 pharmacy residents and students.

Julie Dagam, Pharm.D., BCPS

PGY1 Residency Program Director – Aurora Health Care Metro, Inc Julie received both her B.S. and her Pharm.D. from the University of Wisconsin. She completed a PGY1 Pharmacy Practice Residency at Aurora Health Care, and currently practices at Aurora St. Luke's Medical Center. Julie co-precepts the Epilepsy/Neurology elective learning experience (in the PGY1-Aurora Health Care Metro, Inc program) and the Pharmacy Recruiting required learning experience (in the PGY2 Health System Pharmacy Administration program). She also supports all of the residency programs at sites across the Aurora Health Care system.

Melissa Dahlgren, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Medical and Surgical Pharmacy Services Melissa received her B.S. and Pharm.D. from the University of Minnesota. She completed a specialty residency in nephrology and renal transplant at the Hennepin County Medical Center in Minneapolis, Minnesota. Melissa practices at Aurora St. Luke's Medical Center.

Angela De Ianni, RPh, MBA

Director, Pharmacy Supply Chain Angela graduated from UW-Madison with a B.S. in pharmacy in 1998 and obtained an M.B.A. from UW-Milwaukee in 2005. After working as a staff pharmacist at St. Joseph's Hospital for seven years, she was promoted to Supervisor and then Director of Pharmacy at St. Joseph's for the following three years. Angela came to Aurora in 2009 as the Director of Inpatient Pharmacy at Aurora West Allis Medical Center and took the role of Director, Pharmacy Supply Chain in June 2014.

Lynne Fehrenbacher, Pharm.D., BCPS (AQ-ID)

Lynne Received her Pharm.D. from the University of Wisconsin and completed a PGY1 Pharmacy Practice Residency at Aurora Health Care. Since completion of her residency, Lynne has cared for patients with infections in a range of practice settings from ambulatory HIV care to serving as the pharmacy clinical coordinator for infectious diseases at Aurora St. Luke's Medical Center. As part of her faculty appointment, Lynne practices as part of the ID Pharmacy team at Aurora St. Luke's Medical Center two days per week.

Ashley Feldt, Pharm.D., M.B.A

Director of Pharmacy, Oncology, Aurora St. Luke's Medical Center Ashley received her Pharm.D. and M.B.A. from Drake University in Des Monies, Iowa. She completed her PGY-1 Pharmacy Practice Residency and PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. In her role, she oversees pharmacy services in the oncology service line throughout Aurora. She is involved in precepting the administration rotation.

Jared Frost, PharmD

Clinical Pharmacist Jared received his PharmD from Drake University and completed his PGY-1 Pharmacy Practice and PGY-2 Cardiology residencies at Aurora Health. He now works primarily in the cardiac ICU at Aurora St. Luke's Medical Center. Jared precepts the CICU rotation for the PGY-1 Pharmacy Practice Residency Program and is a CICU rotation preceptor in training for both the PGY-2 Cardiology Pharmacy Residency Program and the PGY-2 Critical Care Pharmacy Residency Program.

Dave Galis, R.Ph.

Clinical Pharmacist Dave received his B.S. in Pharmacy from the University of Wisconsin. He practices in the Operating Room Satellite at Aurora St. Luke's Medical Center and precepts the operating room rotation.

Craig Glienke, Pharm.D., BCPS

Clinical Pharmacist Craig graduated from the University of Minnesota, College of Pharmacy. He completed his PGY1 at Hennepin County Medical Center (HCMC) in Minneapolis, Minnesota. Craig precepts PGY1 residents and PGY2 Critical Care and Cardiology residents in the Cardiovascular Intensive Care Unit.

Jon Godden, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Cardiothoracic Transplant Jon received his Pharm.D. from the University of Iowa and completed his PGY-1 Pharmacy Practice residency at Aurora Health Care. He is now a specialty pharmacy coordinator in cardiothoracic transplant at Aurora St. Luke's Medical Center. Jon precepts the advanced heart failure/cardiothoracic transplant rotation for the PGY-1 Pharmacy Residency Program, the PGY-2 Cardiology Pharmacy Residency Program and the PGY-2 Critical Care Pharmacy Residency Program.

Jennifer Godden, Pharm.D., BCOP

Specialty Pharmacy Coordinator, Oncology Jen received her Pharm.D. from the University of Iowa. She completed her PGY-1Pharmacy Practice and PGY-2 Oncology Specialty Residencies at Aurora Health Care. Jen serves as a preceptor for the PGY-1 outpatient oncology rotation and the PGY-2 oncology specialty residency.

Anne Graff LaDisa, PharmD, BCPS

Anne earned her Doctor of Pharmacy degree from Drake University in Des Moines, Iowa. She completed a post-graduation Pharmacy Practice Residency with Barnes-Jewish Hospital in St. Louis, Missouri. Since completion of her residency, Anne has served as an inpatient clinical pharmacist and inpatient pharmacy clinical coordinator at Aurora Sinai Medical Center before joining the faculty at Concordia University Wisconsin School of Pharmacy. Anne maintains a clinical practice site at Aurora St. Luke's Medical Center where she cares for general surgery and internal medicine inpatients and helps precept Aurora Health Care PGY-1 residents as well as Concordia University Wisconsin's Doctor of Pharmacy students.

Craig Grzendzielewski, Pharm.D., BCPS

Specialty Pharmacy Coordinator Craig received his Pharm.D. from St. Louis College of Pharmacy. He completed a PGY1 Pharmacy Practice residency at Presence Saint Joseph Medical center in Joliet, Il. Craig oversees both the intern program and the medication history program at St. Luke's Medical Center while supporting medication history efforts across the system. Within the residency program, Craig currently precepts year-long resident projects and clinical forums. Craig also serves as a primary preceptor for the PGY1 residents.

Tyler Guetschow, Pharm.D., BCPS

Clinical Pharmacist Tyler received his Pharm.D. from the University of Wisconsin-Madison School of Pharmacy. He completed his PGY1 Pharmacy Practice Residency and PGY2 Critical Care Specialty Residency at Aurora St. Luke's Medical Center. Tyler is the preceptor for the Surgical ICU Rotation at St. Luke's Medical Center.

Steve Hawk, Pharm.D. BCPS

Sr. Clinical Pharmacist Steve received his Pharm.D. from Creighton University in Omaha, Nebraska. He is the preceptor for the Neonatal Intensive Care Unit/Women's Care rotation at Aurora Sinai Medical Center.

Emma Hews, Pharm.D

Director of Pharmacy, Aurora Sinai Medical Center Emma received her BS in Biology/Biotechnology from Worcester Polytechnic Institute and her Pharm.D. from Shenandoah University. She completed both the PGY-1 Pharmacy Practice Residency and the PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care.

Stacy Hilgendorf, Pharm.D., BCPS

Clinical Pharmacist Stacy received her Pharm.D. from the University of Wisconsin and completed her PGY1 Pharmacy Practice Residency at Aurora Health Care. She is a preceptor for the PGY1 Adult Medicine Rotation and has pharmacy experience staffing oncology, general medicine and critical care at St. Luke's Medical Center.

Arlene Iglar, R.Ph., M.S.

Senior Director of Pharmacy Operations Arlene received both her B.S. in Pharmacy and her M.S. in Hospital Pharmacy Administration from the University of Wisconsin. She completed a PGY-1 Pharmacy Practice Residency and a PGY-2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics.

Kelcy Johnson, Pharm.D., BCPS

Clinical Pharmacist Kelcy received her B.S. in Biology from the University of Notre Dame and Pharm.D. from the University of Wisconsin – Madison. She completed her PGY1 with Aurora Health Care. She currently precepts for the Adult Medicine rotation at St. Luke's Medical Center.

Julie L. Kindsfater, Pharm.D., BCPS

Drug Policy, Formulary Coordinator, Sr. Julie received her Pharm.D. from the University of Minnesota – Twin Cities. She completed a PGY-1 Pharmacy Practice Residency at Froedtert Memorial Lutheran Hospital in Milwaukee. Julie co-precepts the drug policy rotation and assists residents with medication safety projects. She chairs the Aurora Medication Safety Committee and recently completed coursework in Patient Safety Leadership through the College of Medicine at the University of Illinois – Chicago.

Arlene Iglar, R.Ph., M.S.

Sr. Director of Pharmacy Operations Arlene received both her B.S. in Pharmacy and her M.S. in Hospital Pharmacy Administration from the University of Wisconsin. She completed a PGY-1 Pharmacy Practice Residency and a PGY-2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics.

Chris Klink, Pharm.D., BCPS

Clinical Pharmacist Senior Chris received his B.S. in pharmacy from the University of Wisconsin at Madison, and his Pharm.D. from the University of Arkansas for Medical Sciences. Chris precepts the Family Practice rotation. He practices at the St. Luke's Family Practice Center and the Aurora Pharmacy in the Aurora St. Luke's Physician Office Building.

Christopher Knock, Pharm.D.

Clinical Pharmacist Christopher received his Pharm.D. from Drake University. He is the preceptor for the Emergency Medicine rotation for the PGY-1 Pharmacy and PGY-2 Critical Care Residency Programs at Aurora St. Luke's Medical Center.

Kerry Koloske, PharmD, BCPS

Clinical Pharmacist Kerry graduated from UW-Madison in 2000. She completed her PGY-1 residency at Aurora St. Luke's Medical Center in 2001 and works as a clinical pharmacist mainly in the Neuro ICU.

Jennifer Lester, PharmD

Clinical Pharmacist Jen received her Pharm.D. from Drake University. She practices in the Central Pharmacy and the Investigational Drug Service at Aurora St. Luke's Medical Center. Jen is co-preceptor of the elective investigational drugs rotation for the PGY-1 Pharmacy and PGY-2 Oncology Residency Programs at Aurora St. Luke's Medical Center.

Sr. Director of Pharmacy Operations Al received his B.S. in Pharmacy from Purdue University. He obtained his M.S. in Hospital Pharmacy Administration from the University of Wisconsin and completed a two-year residency in practice management at the University of Wisconsin Hospital and Clinics.

Erin Lydon, Pharm.D.

Clinical Pharmacist Erin received her Pharm.D. from the University of Iowa. She completed her PGY-1 Pharmacy Residency at Aurora St. Luke's Medical Center. She currently precepts for the PGY-1 Inpatient Oncology Rotation at Aurora St. Luke's Medical Center.

Amy Mahlum, PharmD

PGY1 Community Pharmacy Residency Program Director; Ambulatory Pharmacy Clinical Coordinator Amy completed her pre-pharmacy coursework and her PharmD at the University of Wisconsin-Madison. She completed a PGY-1 Community Pharmacy Residency at Aurora Health Care. She practices both at the Aurora St. Luke's Outpatient Pharmacy and in the Pharmacist-Managed Anticoagulation Clinic. She precepts the clinical services rotation and co-precepts the medication safety rotation.

He completed a PGY-1 Pharmacy Practice Residency with Aurora Health Care. James currently precepts the PGY-1 and PGY-2 Critical Care CICU rotation at St. Luke's Medical Center.

Tracy J. Mathia, Pharm.D., BCPS

Sr. Clinical Pharmacist Tracy received her Pharm.D. from Creighton University in Omaha, Nebraska. She completed a PGY-1 Pharmacy Practice Residency with Aurora Health Care. Since completion of her residency, Tracy has served as an inpatient clinical pharmacist, staffing primarily in cardiac units, and as specialty pharmacy coordinator in cardiothoracic transplant at ASLMC before joining the team at the Packaging Center. Tracy is the coordinator for the Clinical Forum CORE learning experience for the PGY-1 Metro Pharmacy Practice Residency class.

Sarah Matias, PharmD, BCPS

Clinical Pharmacist Sarah received her BS in Biology from Northern Illinois University and her Pharm.D. from the University of Illinois at Chicago. She completed a PGY-1 Pharmacy Practice Residency at Edward Hines Jr. VA Hospital. Sarah works on the surgical and oncology floors and co-precepts these rotations.

He completed his PGY-1 Pharmacy Practice Residency and PGY-2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics. In addition to being the PGY-2 Pharmacy Informatics Program Director, he is a preceptor for the PGY-2 Informatics Residency, PGY-2 Health System Pharmacy Administration Residency, and PGY-1 Pharmacy Residency.

Michael A. Metz, R.Ph.

Mike oversees Central Pharmacy operations, automation and sterile products for Aurora St. Luke's Medical Center. Mike precepts the administration rotation for PGY-1 residents and co-precepts PGY-2 administration residents for operation rotations at Aurora St. Luke's Medical Center.

Sarah Moldenhauer, PharmD

Clinical Pharmacist Sarah received her Pharm.D. from Drake University. She completed her PGY1 Pharmacy Practice Residency with Aurora Health Care. Sarah currently precepts the PGY1 Cardiology rotation at Aurora St. Luke's Medical Center. She is on faculty at Concordia University Wisconsin School of Pharmacy and practices at Aurora St Luke's Medical Center two days per week. Navneet Piton, PharmD

Clinical Pharmacist Navneet received her Pharm.D from the University of Wisconsin. She is co-preceptor for the PGY-1 Neuro/Epilepsy rotation and the Neuro ICU rotation for the PGY-2 critical care residency.

Matt Preuss, R.Ph.

Clinical Pharmacist Matt received is B.S. from the University of Wisconsin. He is a preceptor for the Geriatrics/Parkinson's Disease rotation at Aurora Sinai Medical Center.

Heena Rathod, PharmD

Clinical Pharmacist Heena received her Pharm.D. from the Midwestern University – Chicago College of Pharmacy. She completed her PGY1 Pharmacy Residency and PGY2 Pharmacy Residency in Oncology at Aurora St. Luke's Medical Center. Heena currently co-precept PGY1 inpatient oncology rotation and is a preceptor in training for PGY2 inpatient rotations.

Valerie Ravenna, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Infectious Diseases Valerie received her Pharm.D. from Midwestern University Chicago College of Pharmacy. She completed a PGY-1 Pharmacy Practice residency and PGY-2 Infectious Diseases residency at Meriter Hospital in Madison, WI. Valerie co-precepts both the PGY-1 and PGY-2 infectious diseases rotations.

Suzie Robison-Strane, R.Ph., BCPS

Sr. Clinical Pharmacist Suzie received her B.S. in Pharmacy and completed a residency in nuclear pharmacy at the University of Utah. She practices at the Aurora Pharmacy Central Order Verification Department, is chairperson for the multidisciplinary ASLMC/ASMC MedSafe Committee and co-precepts the ASLMC PGY-1 Pharmacy Resident Medication Safety learning experience.

Lauren Schmidt, PharmD

Clinical Pharmacist Lauren received her Pharm.D. from Drake University. She completed her PGY1 Pharmacy Residency at Aurora Health Care. Lauren currently co-precepts a PGY1 cardiology rotation at Aurora St. Luke's Medical Center.

Clinical Pharmacist PGY1 Pharmacy Residency – Aurora Health Care Metro, Inc. Erin received her Pharm.D. from the University of Wisconsin-Madison. She completed a PGY-1 Pharmacy Practice residency at Aurora St. Luke's Medical Center. Erin is a preceptor for the PGY1 Adult Medicine Rotation and has experience staffing on cardiology, oncology, and surgical acute care units. She is also a member of Aurora Health Care Biomedical Institutional Review Board (IRB).

Ryan Servais, Pharm.D., BCPS

Clinical Pharmacist Ryan received his Pharm.D. from the University of Wisconsin. He completed his PGY-1 Pharmacy and PGY-2 Critical Care Residencies at Aurora Health Care. Ryan precepts the CVICU rotation and also leads topic discussions as part of the critical care, cardiology, ED and OR rotations.

Jessica Small, Pharm.D., BCPP

Director, Pharmacy Services Jessica received her Pharm.D. from the University of Iowa. She completed a psychiatry specialty residency at Western Missouri Mental Health Center in Kansas City, Missouri. Jessica is responsible for both the inpatient and outpatient pharmacy services at Aurora Psychiatric Hospital.

Chad Smith, Pharm.D., MBA, BCPS

Director of Pharmacy, Aurora St. Luke's Medical Center Chad received his BS from Marquette University, and his Pharm.D. and MBA from Creighton University. He did a PGY-1 Pharmacy Practice residency and a specialty PGY-2 Health System Administration residency at St. Luke's Medical Center, Aurora Health Care. Chad precepts the administration rotation for PGY-1 residents and co-precepts PGY-2 administration residents for operation rotations at Aurora St. Luke's Medical Center.

Theresa Smith, PharmD, BCPS

Pharmacy Manager, Centralized Order Verification Theresa received her BS in Health Administration and Policy and her Pharm.D. from Creighton University in Omaha, Nebraska. She completed both the PGY-1 Pharmacy Practice Residency and the PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care.

Stacy Soppe, Pharm.D.

Clinical Pharmacist Stacy received her B.A. from Wartburg College and her Pharm.D. from the University of Iowa. She practices at the Aurora St. Luke's Outpatient Pharmacy and the Aurora St. Luke's Pharmacist-Managed Anticoagulation Clinic. She is the anticoagulation clinic preceptor.

Frank Spexarth, R.Ph., BCPS

Specialty Pharmacy Coordinator, Cardiovascular Surgery Pharmacy Services Frank received his B.S. in Pharmacy from the University of Wisconsin. He practices at Aurora St. Luke's Medical Center and is an ACLS instructor. He co-precepts cardiology, cardiovascular surgery, critical care, and pulmonary arterial hypertension rotations.

Peter Stuessy, Pharm.D., BCPS

Specialty Pharmacy Coordinator Peter received his Pharm.D. from the University of Wisconsin-Madison. He completed his PGY-1 Pharmacy Residency at Aurora St. Luke's Medical Center. He is a primary preceptor and clinical forum preceptor for the PGY-1 program and involved in the Oncology rotations. He is the preceptor for the PGY-2 Specialty Oncology Outpatient Solid Tumor/Heme rotation.

Ryan Van Engel, Pharm.D.

Clinical Pharmacist Ryan received his Pharm.D. from the University of Iowa. He completed his PGY1 Pharmacy Practice Residency at the University of Iowa Hospitals and Clinics and then completed his PGY2 Critical Care Specialty Residency through Midwestern University, practicing at Northwestern Memorial Hospital. At both programs he obtained a teaching certificate. Ryan precepts the SICU rotation for the PGY2 Critical Care residency program, which is where he primarily staffs. He also precepts one of the medical/surgical rotations for the PGY 1 pharmacy residency program.

Angela Volquardsen, Pharm.D., BCPS, CACP

Clinical Pharmacist Angela received her Pharm.D. from the University of Kansas and completed a PGY-1 Pharmacy Practice Residency at Aurora Health Care. Angela has continued involvement with both the Adult Medicine rotation at Aurora St. Luke's Medical Center and the Clinical Teaching rotation.

Specialty Pharmacy Coordinator, System Pharmacy Angie received her Pharm.D. with a concentration in diabetes from Drake University in Des Moines, Iowa. She completed her PGY-1 Pharmacy Practice Residency and PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. Angie is a system pharmacy coordinator and helps precept the Medicine rotation based out of St. Luke's and is a preceptor for the clinical teaching rotation for the PGY1 Pharmacy Residency. Angie also precepts the PGY2 Oncology Pharmacy Resident Medication Safety learning experience.

Jessica Whalen, PharmD, BCPS

Clinical Pharmacist Jessica received her BS in Biology/Chemistry from Illinois College and her PharmD from the University of Iowa College of Pharmacy. She completed a PGY1 residency with the University of Iowa Hospitals and Clinics and a PGY2 critical care residency with Aurora Health Care Metro, Inc. She currently precepts the MRICU rotation for PGY1 residents and the PGY2 critical care resident.

Paul Windisch, Pharm.D.

Specialty Pharmacy Coordinator, Drug Policy Paul received his B.S. in Economics & Statistics from University of Minnesota and his Pharm.D. from the University of Nebraska Medical Center. He completed a PGY-1 Pharmacy Practice Residency at University of Wisconsin Hospital & Clinics, and completed a two-year fellowship in pharmacoeconomics, technology assessment & outcomes research at University Health System Consortium (UHC). Paul co-precepts the drug policy rotation.

Prati Wojtal, R.Ph., M.S.

Director of Ambulatory Pharmacy Services Prati received her B.S. in Pharmacy and M.S. in Hospital Pharmacy Administration from the University of Minnesota. She completed a two-year administrative residency at United and Children's Hospitals in St. Paul, Minnesota. Prati has been very active with the state pharmacy organizations serving first as secretary then as president for the Wisconsin Society of Health-System Pharmacists in 1995-96, and also board member for the Pharmacy Society of Wisconsin in 1998-99 and again in 2006 – 2008. She was appointed to the ASHP Council on Organizational Affairs and the Council on Legal and Professional Affairs, invited to participate as a member of the Consensus Recommendations Panel at the 2014 ASHP Ambulatory Care Conference and Summit, and recognized as a Fellow of ASHP in June 2014

Tom received his B.S. in Pharmacy and M.S. in Hospital Pharmacy Administration from the University of Wisconsin. He completed a two-year residency in practice management at the University of Wisconsin Hospital and Clinics. Tom has served as a member of the ASHP Commission on Credentialing, the body responsible for accreditation of pharmacy residency programs.

Stacy Wucherer, R.Ph.

Manager of Packaging Center Pharmacy Services Stacy received her B.S. in Pharmacy from the University of Wisconsin. She completed the PGY-1 Pharmacy Practice Residency and the PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. Stacy is responsible for the packaging center and pharmacy automation.

Sol Yoder, Pharm.D., BCOP

Sol received her B.S. in Pharmacy from the University of Iowa and her Pharm.D. from the University of Texas Health Science Center at San Antonio. She completed a hematology/oncology specialty residency at the University of Texas MD Anderson Cancer Center in Houston. She precepts the oncology inpatient rotation for both PGY1 and PGY2 residents. She is currently a member of the Board of Pharmacy Specialty (BPS) council for oncology.

Jeff Zimmerman, Pharm.D., BCPS

Specialty Pharmacy Coordinator – Aurora Sinai Medical Center Jeff received his Pharm.D. from Drake University. He completed his PGY1 Pharmacy Residency and his PGY2 Critical Care Residency at Aurora St. Luke's Medical Center in Milwaukee, WI. After three years staffing as a clinical pharmacist in the MRICU at ASLMC, Jeff transitioned to a Specialty Pharmacy Coordinator at Aurora Sinai Medical Center where he is the preceptor for the Internal Medicine rotation.

Nathaniel Zook, Pharm.D., BCPS

Clinical Pharmacist Nate received his Pharm.D. from Philadelphia College of Pharmacy within the University of the Sciences. He completed his PGY-1 Pharmacy Residency at Penn State Milton S. Hershey Medical Center and his PGY-2 Critical Care Specialty Residency at Orlando Regional Medical Center. Nate currently precepts for the PGY-1 and PGY-2 Critical Care MRICU rotation at St. Luke's Medical Center.

Sarah Zukkoor, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Cardiology Sarah received her Bachelors of Science in Human Biology from Michigan State University and her Pharm.D. from the University of Michigan, College of Pharmacy. She completed her PGY1 Pharmacy Residency at the University of Toledo Medical Center and PGY2 Cardiology Pharmacy Residency at the Ohio State University Medical Center. Sarah is a preceptor for acute care cardiology and the Coronary Intensive Care Unit (CICU) rotations at ASLMC.

Nick received his BS in biomedical sciences from Marquette University and his PharmD from the University of Illinois at Chicago. He completed a PGY1 pharmacy practice residency at Aurora St. Luke's Medical Center and a PGY2 pharmacy residency in internal medical at the Zablocki VA Medical Center. Nick is on faculty at Concordia University Wisconsin School of Pharmacy and practices at Aurora St. Luke's Medical Center two days per week. Nick precepts the direct patient care surgical and cardiology rotations as well as the elective clinical teaching rotation.
