Erika Aldag, PharmD, BCPS

Specialty Pharmacy Coordinator, Abdominal Transplant Erika received her PharmD from Drake University. She completed her PGY1 Pharmacy Practice residency at the University of Nebraska Medical Center and herPGY2 Solid Organ Transplant residency at The Ohio State University Medical Center. She precepts both the PGY1 and PGY2 Critical Care Abdominal Transplant rotations.

Todd Armbruster, PharmD

Clinical Pharmacist Todd graduated from Drake University. Todd practices and precepts in the intensive care unit at Aurora Sinai Medical Center.

Sol Atienza, PharmD, BCOP

PGY2 Oncology Pharmacy Residency Program Director
Specialty Pharmacy Coordinator, Oncology Pharmacy Services
Sol received her BS in Pharmacy from the University of Iowa and her PharmD from the University of Texas Health Science Center at San Antonio. She completed a hematology/oncology specialty residency at the University of Texas MD Anderson Cancer Center in Houston. She precepts the oncology inpatient rotation for both PGY1 and PGY2 residents. She is currently the resident program director for the PGY2 Oncology Specialty Residency.

Amanda Birnschein, PharmD, BCPS

Clinical Pharmacist Amanda received her B.A. in psychology at Lawrence University and completed her pre-pharmacy coursework at University of Wisconsin-Milwaukee prior to receiving her PharmD from Thomas Jefferson University. She completed her PGY1 Pharmacy Practice Residency at Aurora Health Care, Metro, Inc. Amanda currently precepts the PGY1 residents for the Internal Medicine Rotation at Aurora Sinai Medical Center.

Dennis Brierton, PharmD, BCPS, FASHP

System Director, Clinical Services Dennis received his B.S. from the University of Wisconsin and his PharmD from the University of Illinois-Chicago. He completed a specialty residency in adult internal medicine at Purdue University.

Emily Bryant, PharmD, BCPS

Clinical Pharmacist Emily completed pre-pharmacy coursework at the University of Wisconsin prior to receiving her PharmD from Concordia University Wisconsin. She completed her PGY1 Pharmacy Residency at Aurora St. Luke's Medical Center. She is the preceptor for the Emergency Medicine rotation for the PGY1 Pharmacy and PGY2 Critical Care Residency Programs at Aurora St. Luke's Medical Center.

Jane Bubik, PharmD

Investigational Drugs Coordinator Jane received both her B.S. and PharmD from Purdue University. She completed a PGY1 Pharmacy Practice Residency at Rush-Presbyterian St. Luke's Medical Center in Chicago. Jane offers an elective investigational drugs rotation at St. Luke's Medical Center.

Angela Colella, PharmD, BCPS

Margaret Cook, PharmD, BCPS

Specialty Pharmacy Coordinator Margaret received her B.S. from the University of Wisconsin and her PharmD from the University of North Carolina. She completed a PGY1 Pharmacy Practice Residency at Hartford Hospital, Harford, Connecticut. Margaret practices at Aurora St. Luke's Medical Center. She co-precepts rotations in infectious diseases for PGY1 and PGY2 pharmacy residents and students.

Sarah Cullen, PharmD, BCPS

Clinical Pharmacist Sarah received her BS in Biology from Northern Illinois University and her PharmD from the University of Illinois at Chicago. She completed a PGY1 Pharmacy Practice Residency at Edward Hines Jr. VA Hospital. Sarah works on the surgical and oncology floors and co-precepts these rotations.

Julie Dagam, PharmD, BCPS

PGY1 Residency Program Director – Aurora Health Care Metro, Inc Julie received both her B.S. and her PharmD from the University of Wisconsin. She completed a PGY1 Pharmacy Practice Residency at Aurora Health Care, and currently practices at Aurora St. Luke's Medical Center. Julie co-precepts the Epilepsy/Neurology elective learning experience (in the PGY1-Aurora Health Care Metro, Inc program) and the Pharmacy Recruiting required learning experience (in the PGY2 Health System Pharmacy Administration program). She also supports all of the residency programs at sites across the Aurora Health Care system.

Melissa Dahlgren, PharmD, BCPS

Specialty Pharmacy Coordinator, Medical and Surgical Pharmacy Services Melissa received her B.S. and PharmD from the University of Minnesota. She completed a specialty residency in nephrology and renal transplant at the Hennepin County Medical Center in Minneapolis, Minnesota. Melissa practices at Aurora St. Luke's Medical Center.

Kasey Davis, PharmD, BCPS

Clinical Pharmacist Kasey received her PharmD from the University of Wisconsin-Madison. She completed her pharmacy practice residency at the University Medical Center Brackenridge in Austin, TX with a focus in cardiology and anticoagulation. Kasey precepts the PGY2 Heart Failure Rotation.

Tom Dilworth, PharmD, BCPS-AQ ID

Specialty Pharmacy Coordinator, Infectious Diseases Tom received his B.A. in Spanish Language and Culture from the University of Wisconsin-Milwaukee and his PharmD from the University of Wisconsin. He completed a PGY1 pharmacy practice residency, a PGY2 infectious diseases residency, and a postdoctoral pharmacy fellowship in infectious diseases at the University of New Mexico Health Sciences Center. Tom co-precepts both the PGY1 and PGY2 infectious diseases rotations at Aurora St. Luke's Medical Center. His clinical research focuses on optimizing antibiotic therapy for patients with invasive gram-positive infections and measuring patient and provider-level factors associated with antiretroviral therapy adherence.

Lynne Fehrenbacher, PharmD, BCPS (AQ-ID)

Lynne Received her PharmD from the University of Wisconsin and completed a PGY1 Pharmacy Practice Residency at Aurora Health Care. Since completion of her residency, Lynne has cared for patients with infections in a range of practice settings from ambulatory HIV care to serving as the pharmacy clinical coordinator for infectious diseases at Aurora St. Luke's Medical Center. As part of her faculty appointment, Lynne practices as part of the ID Pharmacy team at Aurora St. Luke's Medical Center two days per week.

Chris Forberg, RPh

Clinical Pharmacist Chris completed his Associate in Science degree from Rock Valley College and his BSPharm from the University of Wisconsin-Madison. He practices pharmacy full time at the Aurora St. Luke's Outpatient Pharmacy and the Aurora St. Luke's Pharmacist-Managed Anticoagulation Clinic. He is currently credentialed as a specialist in the management of anticoagulation in an outpatient setting. He serves as a primary preceptor for the community-based residency program.

Jared Frost, PharmD

Clinical Pharmacist Jared received his PharmD from Drake University and completed his PGY1 Pharmacy Practice and PGY2 Cardiology residencies at Aurora Health Care. He now works primarily in the cardiac ICU at Aurora St. Luke's Medical Center. Jared precepts the CICU rotation for the PGY1 Pharmacy Practice Residency Program, the PGY2 Cardiology Pharmacy Residency Program, and the PGY2 Critical Care Pharmacy Residency Program.

Dave Galis, RPh

Clinical Pharmacist Dave received his B.S. in Pharmacy from the University of Wisconsin. He practices in the Operating Room Satellite at Aurora St. Luke's Medical Center and precepts the operating room rotation.

Craig Glienke, PharmD, BCPS, BCCCP

Clinical Pharmacist Craig graduated from the University of Minnesota, College of Pharmacy. He completed his PGY1 at Hennepin County Medical Center (HCMC) in Minneapolis, Minnesota. Craig precepts PGY1 residents and PGY2 Critical Care and Cardiology residents in the Cardiovascular Intensive Care Unit.

Jennifer Godden, PharmD, BCOP

Specialty Pharmacy Coordinator - Oncology Pharmacogenomics Jen received her PharmD from the University of Iowa. She completed her PGY1 Pharmacy Practice and PGY2 Oncology Specialty Residencies at Aurora Health Care. She is currently working as the Co-Director for the Oncology Precision Medicine program. Jen serves as a primary preceptor for the PGY1 pharmacy practice residency and a rotation preceptor for the PGY2 oncology specialty residency.

He is now a specialty pharmacy coordinator in cardiothoracic transplant at Aurora St. Luke's Medical Center. Jon precepts the advanced heart failure/cardiothoracic transplant rotation for the PGY1 Pharmacy Residency Program, the PGY2 Cardiology Pharmacy Residency Program and the PGY2 Critical Care Pharmacy Residency Program.

Anne Graff LaDisa, PharmD, BCPS

Anne earned her Doctor of Pharmacy degree from Drake University in Des Moines, Iowa. She completed a post-graduation Pharmacy Practice Residency with Barnes-Jewish Hospital in St. Louis, Missouri. Since completion of her residency, Anne has served as an inpatient clinical pharmacist and inpatient pharmacy clinical coordinator at Aurora Sinai Medical Center before joining the faculty at Concordia University Wisconsin School of Pharmacy. Anne maintains a clinical practice site at Aurora St. Luke's Medical Center where she cares for general surgery and internal medicine inpatients and helps precept Aurora Health Care PGY1 residents as well as Concordia University Wisconsin's Doctor of Pharmacy students.

Tyler Guetschow, PharmD, BCPS

Clinical Pharmacist Tyler received his PharmD from the University of Wisconsin-Madison School of Pharmacy. He completed his PGY1 Pharmacy Practice Residency and PGY2 Critical Care Specialty Residency at Aurora St. Luke's Medical Center. Tyler is the preceptor for the Surgical ICU Rotation at St. Luke's Medical Center.

Mark Hamm, PharmD, MBA, BCPS

Director of Pharmacy, Aurora St. Luke's Medical Center Mark received his PharmD and MBA from Creighton University in Omaha, Nebraska. He completed a PGY1 Pharmacy Practice Residency and PGY2 Health-System Pharmacy Administration Residency at Saint Luke's Hospital of Kansas City. Mark oversees pharmacy services in the oncology service line throughout Aurora and precepts residents on administration rotations.

Steve Hawk, PharmD, BCPS

Sr. Clinical Pharmacist Steve received his PharmD from Creighton University in Omaha, Nebraska. He is the preceptor for the Neonatal Intensive Care Unit/Women's Care rotation at Aurora Sinai Medical Center.

Emma Hews, PharmD, MBA

Director of Pharmacy, Aurora Sinai Medical Center Emma received her B.S. in Biology/Biotechnology from Worcester Polytechnic Institute, her PharmD from Shenandoah University and her MBA from Concordia University. She completed both the PGY1 Pharmacy Practice Residency and the PGY2 Health-System Pharmacy Administration Residency at Aurora Health Care. Emma currently precepts the administration rotations for both the PGY1 Pharmacy and PGY2 Administration programs.

Stacy Hilgendorf, PharmD, BCPS

Clinical Pharmacist Stacy received her PharmD from the University of Wisconsin and completed her PGY1 Pharmacy Practice Residency at Aurora Health Care. She is a preceptor for the PGY1 Adult Medicine Rotation and has pharmacy experience staffing oncology, general medicine and critical care at St. Luke's Medical Center.

Arlene Iglar, RPh, M.S.

Sr. Director of Pharmacy Operations Arlene received both her B.S. in Pharmacy and her M.S. in Hospital Pharmacy Administration from the University of Wisconsin. She completed a PGY1 Pharmacy Practice Residency and a PGY2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics.

Julie L. Kindsfater, PharmD, BCPS

Medication Safety Program Manager Julie received her PharmD from the University of Minnesota - Twin Cities. She completed a PGY1 Pharmacy Practice Residency at Froedtert Memorial Lutheran Hospital in Milwaukee. Julie co-precepts the drug policy rotation and assists residents with medication safety projects. She chairs the Aurora Medication Safety Committee and recently completed coursework in Patient Safety Leadership through the College of Medicine at the University of Illinois - Chicago.

Chris Klink, PharmD, BCPS

Clinical Pharmacist Senior Chris received his B.S. in pharmacy from the University of Wisconsin at Madison, and his PharmD from the University of Arkansas for Medical Sciences. Chris precepts the PGY1 Family Practice rotation, the PGY1 Community Residency longitudinal immunization rotation, and is a PGY1 project preceptor. He practices at the St. Luke's Family Practice Center and the Aurora Pharmacy in the Aurora St. Luke's Physician Office Building.

Angie Knutson, PharmD, BCPS

Specialty Pharmacy Coordinator, System Pharmacy Angie received her PharmD with a concentration in diabetes from Drake University in Des Moines, Iowa. She completed her PGY1 Pharmacy Practice Residency and PGY2 Health-System Pharmacy Administration Residency at Aurora Health Care. Angie is a system pharmacy coordinator and is one of the preceptors for the clinical teaching rotation and the medication safety longitudinal rotation for the PGY1 Pharmacy Residency Program. Angie also precepts the PGY2 Oncology, Critical Care, and Infectious Diseases Pharmacy Resident medication safety learning experiences.

Kerry Koloske, PharmD, BCPS

Clinical Pharmacist Kerry graduated from UW-Madison in 2000. She completed her PGY1 residency at Aurora St. Luke's Medical Center in 2001 and works as a clinical pharmacist mainly in the Neuro ICU.

Jennifer Lester, PharmD

Mike oversees Central Pharmacy operations, automation and sterile products for Aurora St. Luke's Medical Center. Mike precepts the administration rotation for PGY1 residents and co-precepts PGY2 administration residents for operation rotations at Aurora St. Luke's Medical Center.

Clinical Oncology Pharmacist Laura received both her B.S. in biochemistry and her PharmD from the University of Wisconsin-Madison. She completed her PGY1 residency and PGY2 residency in oncology at Aurora St. Luke's Medical Center. She currently practices in the Aurora Cancer Care South Clinic. She co-precepts rotations in the oncology clinic for PGY1 and PGY2 pharmacy residents and students, and also co-precepts the longitudinal Neurologic Oncology Rotation for PGY2 pharmacy residents in oncology.

Chad Smith, PharmD, MBA, BCPS

Director of Pharmacy, Aurora St. Luke's Medical Center Chad received his BS from Marquette University, and his PharmD and MBA from Creighton University. He did a PGY1 Pharmacy Practice residency and a specialty PGY2 Health System Administration residency at St. Luke's Medical Center, Aurora Health Care. Chad precepts the administration rotation for PGY1 residents and co-precepts PGY2 administration residents for operation rotations at Aurora St. Luke's Medical Center. Chad is the personnel supervisor of all the PGY1 Pharmacy Practice Residents.

Theresa Smith, PharmD, BCPS

Pharmacy Manager, Centralized Order Verification Theresa received her BS in Health Administration and Policy and her PharmD from Creighton University in Omaha, Nebraska. She completed both the PGY1 Pharmacy Practice Residency and the PGY2 Health-System Pharmacy Administration Residency at Aurora Health Care. Frank Spexarth, RPh, BCPS

Specialty Pharmacy Coordinator, Cardiovascular Surgery Pharmacy Services Frank received his B.S. in Pharmacy from the University of Wisconsin. He practices at Aurora St. Luke's Medical Center and is an ACLS instructor. He co-precepts cardiology, cardiovascular surgery, critical care, and pulmonary arterial hypertension rotations.

Peter Stuessy, PharmD, BCPS,BCOP

He is a primary preceptor and clinical forum preceptor for the PGY1 program and involved in the Oncology rotations. He is the preceptor for the PGY2 Specialty Oncology Outpatient Solid Tumor/Heme rotation.

Ryan Van Engel, PharmD, BCCCP

Clinical Pharmacist Ryan received his PharmD from the University of Iowa. He completed his PGY1 Pharmacy Practice Residency at the University of Iowa Hospitals and Clinics and then completed his PGY2 Critical Care Specialty Residency through Midwestern University, practicing at Northwestern Memorial Hospital. At both programs he obtained a teaching certificate. Ryan precepts the SICU rotation for the PGY2 Critical Care residency program, which is where he primarily staffs. He also precepts one of the medical/surgical rotations for the PGY 1 pharmacy residency program.

Angela Volquardsen, PharmD, BCPS, CACP

Clinical Pharmacist Angela received her PharmD from the University of Kansas and completed a PGY1 Pharmacy Practice Residency at Aurora Health Care. Angela has continued involvement with both the Adult Medicine rotation at Aurora St. Luke's Medical Center and the Clinical Teaching rotation.

Jessica Whalen, PharmD, BCPS, BCCCP

Clinical Pharmacist Jessica received her BS in Biology/Chemistry from Illinois College and her PharmD from the University of Iowa College of Pharmacy. She completed a PGY1 residency with the University of Iowa Hospitals and Clinics and a PGY2 critical care residency with Aurora Health Care Metro, Inc. She currently precepts the MRICU rotation for PGY1 residents and the PGY2 critical care resident.

Paul Windisch, PharmD

Specialty Pharmacy Coordinator, Drug Policy Paul received his B.S. in Economics & Statistics from University of Minnesota and his PharmD from the University of Nebraska Medical Center. He completed a PGY1 Pharmacy Practice Residency at University of Wisconsin Hospital & Clinics, and completed a two-year fellowship in pharmacoeconomics, technology assessment & outcomes research at University Health System Consortium (UHC). Paul co-precepts the drug policy rotation.

Prati Wojtal, RPh, M.S., FASHP

Director of Ambulatory Pharmacy Services Prati received her B.S. in Pharmacy and M.S. in Hospital Pharmacy Administration from the University of Minnesota. She completed a two-year administrative residency at United and Children's Hospitals in St. Paul, Minnesota. Prati has been very active with the state pharmacy organizations serving first as secretary then as president for the Wisconsin Society of Health-System Pharmacists in 1995-96, and also board member for the Pharmacy Society of Wisconsin in 1998-99 and again in 2006 – 2008. She was appointed to the ASHP Council on Organizational Affairs and the Council on Legal and Professional Affairs, invited to participate as a member of the Consensus Recommendations Panel at the 2014 ASHP Ambulatory Care Conference and Summit, and recognized as a Fellow of ASHP in June 2014

Tom Woller, RPh, M.S., FASHP

Tom received his B.S. in Pharmacy and M.S. in Hospital Pharmacy Administration from the University of Wisconsin. He completed a two-year residency in practice management at the University of Wisconsin Hospital and Clinics. Tom has served as a member of the ASHP Commission on Credentialing, the body responsible for accreditation of pharmacy residency programs.

Stacy Wucherer, RPh

Manager of Packaging Center Pharmacy Services Stacy received her B.S. in Pharmacy from the University of Wisconsin. She completed the PGY1 Pharmacy Practice Residency and the PGY2 Health-System Pharmacy Administration Residency at Aurora Health Care. Stacy is responsible for the packaging center and pharmacy automation.

Jeff Zimmerman, PharmD, BCPS

Specialty Pharmacy Coordinator – Aurora Sinai Medical Center Jeff received his PharmD from Drake University. He completed his PGY1 Pharmacy Residency and his PGY2 Critical Care Residency at Aurora St. Luke's Medical Center. He currently oversees all residents at Aurora Sinai Medical Center, acts as a primary preceptor and precepts year-long projects.

Nathaniel Zook, PharmD, BCPS, BCCCP

Clinical Pharmacist Nate received his PharmD from Philadelphia College of Pharmacy within the University of the Sciences. He completed his PGY1 Pharmacy Residency at Penn State Milton S. Hershey Medical Center and his PGY2 Critical Care Specialty Residency at Orlando Regional Medical Center. Nate currently precepts for the PGY1 and PGY2 Critical Care MRICU rotation at St. Luke's Medical Center.

Sarah Zukkoor Zorn, PharmD, BCPS-AQ Cardiology

Pharmacy Clinical Coordinator, Cardio-oncology; PGY2 Pharmacy Residency Director, Cardiology Sarah received her Bachelors of Science in Human Biology from Michigan State University and her PharmD from the University of Michigan, College of Pharmacy. She completed her PGY1 Pharmacy Residency at the University of Toledo Medical Center and PGY2 Cardiology Pharmacy Residency at the Ohio State University Medical Center. Sarah is a PGY1 preceptor for clinical forum and leads topic discussions in cardiology. She is the PGY2 Pharmacy Residency Director and precepts the electrophysiology and cardiac coordination rotations for the PGY2 Cardiology program. She also precepts year-long projects for both programs.

Nick Zupec, PharmD, BCPS

Assistant Professor, Department of Pharmacy Practice, Concordia University Wisconsin School of Pharmacy Nick received his B.S. in biomedical sciences from Marquette University and his PharmD from the University of Illinois at Chicago. He completed a PGY1 pharmacy practice residency at Aurora St. Luke's Medical Center and a PGY2 pharmacy residency in internal medical at the Zablocki VA Medical Center. Nick is on faculty at Concordia University Wisconsin School of Pharmacy and practices at Aurora St. Luke's Medical Center two days per week. Nick precepts the direct patient care medicine and surgery rotations and the elective clinical teaching rotation.
