Enrollment process

Application overview

Application forms will be accepted October – December each year. All applications must be postmarked by December 31st. No applications will be accepted if postmarked after this date.

A $50.00 non-refundable application fee is required. All payments should be made by personal check or money order payable to Aurora St. Luke’s Medical Center.

Instructions on submitting an application

Please note, class size is limited and not all applicants are selected for admission to the program. Applicants who are selected for interviews will be notified by mail.

All program pre-requisites must be completed before an application is submitted.

Failure to meet any of the application requirements will result in loss of application eligibility.

Admission Requirements:

  1. All applicants must meet the personal requirements listed on the website.
  2. All applicants must comply with one of the prerequisites listed on the website.
    • Applicants are encouraged to complete all prerequisites by the end of the enrollment period.
    • Prerequisite 1 applicants must complete their Allied Health program prior to the start of the sonography program in the fall.
    • Prerequisite 2 applicants must complete all program prerequisites prior application.
    • Prerequisite 3 applicants must complete all prerequisites and be in good standing prior to the start of the program.
    • Applicants must achieve a minimum GPA on mandatory coursework as defined by the program.
  3. All applicants must complete a job shadowing experience
    • Each candidate will complete a minimum of four (4) hours in cardiac AND a minimum of four (4) hours in general/vascular
    • Acceptable sites for job shadowing experience are a hospital or medical center
    • Job shadow forms are available on the website; completed forms should be submitted with the application
  4. Two professional references including the reference check list along with a formal written letter
    • College professors, health care professionals and current managers or supervisors are considered appropriate professional references.
  5. All applicants will possess current direct patient care experience. Applicants must have the Verification of Patient Care form completed by their current employer and return the form with their application materials. Failure to submit the required documentation will result in an incomplete application.
    • Prerequisite 1 applicant must possess a minimum of 200 hours direct patient care experience. Experience may be obtained in one of the following ways:
      • Within current healthcare career
      • Clinical experience not including proficiency or clinical lab demonstration hours while attending allied health education program
    • Prerequisite 2 & 3 applicants must complete a certified nursing course and obtain a minimum of 100 hours of direct patient care in addition to the total hours for the CNA course. Current direct patient care experience as a certified nursing assistant (CNA) is obtained in either a hospital or nursing home setting.
  6. All applicants will submit a brief autobiography covering the past four years of the applicant's life. The narrative will state why he/she has an interest in the field of sonography and Aurora St. Luke's Medical Center School of Diagnostic Medical Sonography.

Submitting transcripts

Official Academic transcripts from high school and post-secondary educational institutions are required to document the completion of pre-requisites. To maintain eligibility, official sealed academic transcripts must be received and postmarked by application deadline. Transcripts should be sent to the program directly from the educational institution.

Hand carried transcripts will not be accepted.

General Eligibility Rquirements

All required information must be submitted by the stated due-dates. Failure to meet all deadlines will result in a rejection of the application. In addition, official transcripts must verify all educational information provided. Any discrepancies will terminate an applicant's eligibility.

As a condition of admission, applicants considered for an appointment will be required to pass a drug screen and physical examination. The costs of said tests are the responsibility of the sponsoring institution.
